Diabetic retinal exams
Diabetic retinopathy is the leading cause of preventable blindness in working-age adults, and about 95% of that vision loss is preventable with early detection. Members who are homebound, have limited mobility, or simply won't visit an eye specialist are the ones who miss the annual exam — and the ones we reach.

Bone mineral density testing
Osteoporosis affects roughly one in five women over 50 and typically shows no symptoms until a bone breaks. The OMW measure looks for testing or treatment after a fracture — a narrow window that's easy to miss when the member has to travel to get scanned.

Which measures do the programs address?
Diabetic retinal exams support the HEDIS Eye Exam for Patients with Diabetes (EED) measure. Bone density testing supports Osteoporosis Management in Women Who Had a Fracture (OMW). Blood pressure checks support Controlling High Blood Pressure (CBP). Findings are also documented to support risk adjustment.
How does reporting reach our systems?
Results are delivered through VueConnect with CPT, CPT II, ICD-10, and HCC-mapped codes. We can automate CPT II submissions for applicable measures and provide flat-file or dashboard access, on a weekly cadence or a schedule you set.
Where do you operate?
Screening programs currently run in 48 states. Portable radiology and cardiology (X-ray, ultrasound, EKG) is available in Alabama and Florida. Ask us about coverage for a specific member population.
What does a member experience?
Outreach begins with a letter and call from VuePoint on behalf of the plan. We verify identity using three data points from the plan's file and never request Social Security or banking details. Schedulers work Monday–Friday, 8 AM–6 PM Central. The in-home visit takes about 10–15 minutes and the member pays nothing.
Can screenings be delivered in clinics or at events?
Yes. We run in-home visits, in-clinic days at provider offices, and community events. Plans commonly combine models to reach different segments of a population under a single contract.
How is data protected?
VueConnect and our Microsoft 365 environment, hosted on Microsoft Azure, hold HITRUST i1 certification, renewed in April 2026. The 2026 certification also covers our corporate office facilities.
